This is a list of items you will need to complete your enrollment process (listed in order of importance):

1. Proof of Residency consisting of a: deed, building permit, rental agreement, voter registration card, or utility bill.
2. Original Birth Certificate (not a copy or hospital record)
3. Immunization Records
4. Address and Zip Code of previous school
5. Custody (if applicable)
6. Social Security Number
7. Current Report Card
8. Standardized Test Scores (7th and 8th grade)
